1. Basic concepts
1.1 Questionnaire – the questionnaire for the OPTIKOS PASAULIS Card, in which the Customer provides his/her Personal Data and confirms his/her consent to its processing in whole or in part, and confirms that he/she is familiar with these Terms and Conditions, and on the basis of which the Card shall be issued and activated.
1.2 Personal Data means the personal data of the Cardholder provided by the Cardholder when completing the Questionnaire and the history of the Cardholder’s purchases with the Card, provided that the processing of such data is subject to the requirements of the Law of the Republic of Lithuania on the Protection of Data of the Cardholder.

1.4. Card – the card of the OPTIKOS WORLD, issued to the Company’s customers in accordance with the procedure set out in these Rules.
1.5. Cardholder – the Company’s customer, to whom the Card has been issued.Loyalty Programme – a customer loyalty programme applied by the Company, under which the Cardholder may receive special discounts and/or other exclusive offers in the Company’s store in accordance with the procedures set out in these Terms and Conditions.
1.7. Terms and Conditions – the Rules and Regulations of this OPTICS WORLD CARD.

2. General provisions
2.1 These Terms and Conditions set out the general terms and conditions for the use of the Loyalty Programme benefits and the Card. Relationships which are not regulated by these Terms and Conditions, or which are only partially regulated by these Terms and Conditions, shall be governed separately by the Company’s public notices to customers and by the legislation of the Republic of Lithuania.
2.2. By completing and submitting the Questionnaire, the Cardholder confirms that he/she is duly acquainted with these Terms and Conditions and undertakes to abide by them. The Company shall not be liable for the Cardholder’s failure to read the Terms and Conditions in whole or in part, even though the Cardholder has been given the opportunity to do so.
2.3. The Card is the property of the Company. The Card may be blocked and/or taken away from the Cardholder at any time if it is used in violation of these Terms and Conditions or the legislation of the Republic of Lithuania.
2.4. The Company shall have the right to change the Terms and Conditions or terminate the Loyalty Programme at any time, in whole or in part, without any prior personal notice to Cardholders. Cardholders shall be informed of such decisions at the Optical World Opticians’ showrooms.
2.5. When the Cardholder presents the Card at the Optical World Opticians’ showrooms, the Terms and Conditions in force at the time of presentation of the Card shall apply.
3. Purchasing a card
3.1 The Card may be purchased by natural persons who have completed the Questionnaire and have provided their Personal Data, as specified in the Questionnaire, as mandatory. The mandatory submission of Personal Data is necessary for the Company to properly administer the Loyalty Programme. Failure to provide the required data will result in the Card not being issued.
3.2 The Cardholder undertakes to provide accurate, correct and complete Personal Data in the Questionnaire. In the event of a change in the Personal Data provided by the Cardholder, the Cardholder may inform the Company. The Company shall not be liable for any damage suffered by the Cardholder and/or third parties as a result of the Cardholder’s provision of incorrect and/or incomplete Personal Data or failure to amend and complete the Personal Data after the Personal Data has changed. If the Cardholder provides Personal Data of third parties when completing the Questionnaire, the Cardholder shall be responsible for the lawfulness of the provision and use of such data.
3.3. The Company’s Customer shall complete and submit the Questionnaire at any of the Company’s Opticians when purchasing the Card.
3..4 The Card shall be issued to the Company’s Customer upon presentation of a duly completed and signed Questionnaire and payment of a Card issuance fee of four euro and ninety cents, if applicable at the time of issuance of the Card.
3.5 The Cardholder may only have one Card issued in his/her name. If the Cardholder loses, damages or otherwise misplaces the Card and wishes to replace a valid Card with a new one, the Card shall be reissued upon resubmission of the completed Questionnaire and payment of the Card Issuance Fee and the Card previously issued to the Cardholder shall be cancelled.

4.1. When making purchases at the OPTICS WORLD showrooms, the Cardholder shall receive special discounts and/or other exclusive offers in accordance with the procedures established by the Company.
4. Loyalty programme benefits and conditions of use
4.1.When making purchases at the OPTIKOS PASAULIS showrooms, the Cardholder shall receive special discounts and/or other exclusive offers in accordance with the procedures established by the Company.
5. Validity of the card
5.1 The Card shall be valid for 2 years from the date of issue or until the occurrence of one of the following circumstances:
5.1.1. if the Cardholder loses, damages or otherwise misplaces the Card and requests that the Card be blocked;
5.1.2. if the Company blocks the Card due to the non-provision or incomplete provision of the data and/or consents required to be provided in the Questionnaire, or due to a suspected case of forgery, unauthorised use or fraud of the Card;
5.1.3. in the event of a request by the Cardholder to cease the processing of his/her Personal Data for the purposes of the administration of the Loyalty Programme and/or refusal to participate in the Loyalty Programme;
5.1.4. in the event of the cancellation by the Company of the Loyalty Programme and of the validity of all the Cards.
5.2 Upon expiry of the Card, the Card shall be blocked and the Cardholder’s Personal Data shall be deleted without prior notice to the Cardholder.
6. Processing of personal data
6.1 The Cardholder’s Personal Data shall be processed, stored and used in accordance with the Law on Legal Protection of Personal Data of the Republic of Lithuania. The Company shall be the controller of the Cardholder’s Personal Data. The Company confirms that the State Data Protection Inspectorate of the Republic of Lithuania has carried out a preliminary check in respect of the Company and has granted it permission to process the Cardholder’s Personal Data for the purposes set out in these Terms and Conditions.
6.2 By submitting the completed Questionnaire, the Cardholder agrees to have his/her Personal Data processed for the purpose of administering the Loyalty Programme. The Cardholder grants consent to the Company by signing the Questionnaire. If a person does not consent to the processing of his/her Personal Data for the purposes set out in this clause, the Questionnaire shall not be accepted and the Card shall not be issued.
6.3. The Cardholder’s consent to the processing of his/her Personal Data for direct marketing purposes means that he/she consents to the Company formulating and sending special offers, advertising and other information to the Cardholder on the basis of such data, in relation to the activities of the Opticians and the goods sold therein.
6.4. For the purpose of direct marketing, only the Personal Data that allow the creation of special offers for the Cardholder or a specific group of Cardholders, the identification of the Cardholder and the provision of direct marketing messages to the Cardholder are processed.
6.5 The Company reserves the right to ask the Cardholder to reconfirm the contact details provided in the Questionnaire by text message by telephone and/or email.
6.6 The Cardholder’s Personal Data shall be kept for a maximum period of two years after the last use of the Card. After the expiry of this period, the Cardholder’s Personal Data shall be deleted without prior notice to the Cardholder.
6.7. The Personal Data provided by the Cardholder will be used exclusively by the Company and its partners with whom the Company cooperates in the administration of the Loyalty Programme. The Company will not disclose the Cardholder’s Personal Data to third parties other than the Company’s partners referred to above in this clause, or where obliged to do so by the legislation of the Republic of Lithuania.
6.9. The Cardholder shall have the right to submit a request to the Company at any time to have his/her Personal Data cease to be processed for the purposes of direct marketing by writing to the Company by post, at the following e-mail address [email protected] or by delivering the request directly to the Company.
7. Final provisions
7.1 These Terms and Conditions are made in accordance with the legislation of the Republic of Lithuania. Any dispute, controversy or claim arising out of or in connection with these Terms and Conditions and the breach or validity of these Terms and Conditions shall be settled by negotiation and, in the event of failure to reach an agreement, shall be finally settled by a court of law of the Republic of Lithuania, in accordance with the laws in force in the Republic of Lithuania.
